For many individuals and families, taking the first steps toward starting Care Coordination Services can seem both frightening and daunting. Our Intake Department often receives calls from people who are overwhelmed with emotion after learning their child or loved one’s diagnosis. Intake is there to help them know where to begin.

“I have heard people say they were told to call us by their doctor, a friend, or the Office for People with Developmental Disabilities,” says Intake Manager Lisa Darrow. “Sometimes, people do not really know or understand why they are calling. Working with a Care Coordinator is without a doubt one of the greatest supports they could find, but getting to that point is not always quick or easy. The assistance our caring and diligent Intake Department members can offer is incredibly valuable to individuals and families and is one major and meaningful answer to that ‘Why am I calling?’ question.”

During the first days of the Intake process, there is paperwork, testing, evaluations, and more. Person Centered Services’ Intake Specialists are there to help with all of this. From the initial referral to full enrollment, families will feel supported as they work with an Intake Specialist to complete the steps to obtain Care Coordination Services. Our team is experienced, well-trained, and has the knowledge and expertise to help families navigate through each part of the process.

“Our Intake Specialist was wonderful and explained everything to me,” says Nichole Davis, the mother of Caitlyn, a child who now benefits from Care Coordination support. “This has been the easiest part of getting care for my daughter.”
