You can find opportunities in your region by using the Disability Services NY Directory. This platform offers individuals with disabilities and their families a centralized resource to find trusted medical providers and community supports throughout New York State.
Disability Services NY was developed through a collaboration between the New York State Department of Health (NYS DOH), and three New York State Care Coordination Organizations – ACANY, LIFEPlan CCO NY, and Person Centered Services. Combined, the three Care Coordination Organizations cover the entire state of New York and provide Care Management services to nearly 70,000 individuals.
This innovative platform simplifies the search for essential services such as: day programs, employment support, general healthcare, and other community resources.
Users can search by service type or specialty, organization name, or county, with advanced options to filter for accessibility features like parking, building navigation, telehealth availability, and staff experience with developmental disabilities.
