New Member & Provider Portal Launching in 2024

Learn more about the portal for members and providers scheduled to go-live in September.

 

The new Member & Provider Portal will help our members receive more effective and coordinated services in a way that improves their quality of life. It will also support service delivery for providers that partner with Person Centered Services.

 

Benefits for Our Members

– Receive increased access to additional information from all service providers in a better coordinated way between the providers and Person Centered Services.

– Foster stronger relationships between Care Coordinators and the people they serve

– Offer a convenient way to exchange information, ask questions, and review life plans.

 

Benefits for Providers

– More efficient workflow for staff when engaging people with intellectual and developmental disabilities supported by Person Centered Services.

– Improved access to additional information about the people to whom providers deliver services.

– Direct access to Person Centered Services through secure messaging.

 

Key Portal Functions for Members and Providers

– Providers will be able to view, download, acknowledge, agree to, finalize and electronically sign life plans.

– Secure messages can be sent between providers and Person Centered Services.

– Uploading and storage for documents and staff action plans will be available.

– View member’s Care Team Information. Care Coordinator and Supervisor name and contact information.

– Access to, our member’s life plan(s). Historical and Current.

– Securely upload and download documents for our members.

– Send a direct message to our member, member representative, their Care Coordinator and listing of providers

 

What can you do now to prepare for the new Member & Provider Portal?

Members: Nothing yet, but details about when and how the new portal can be of benefit will be shared when it becomes available.

Providers: Accessing and utilizing the portal will require coordination by providers among IT, HR, QA and other possible staff.

Providers are encouraged to consider the roles within their organization that will be involved in establishing access and then ongoing utilization of the portal.

Trainings, demonstrations and other support needed to establish access and utilize all the functions of the portal will be provided by Person Centered Services.

 

What to expect next?

In July, this webpage will be updated with training schedule of dates and times for portal training, including about where and how to establish access to the portal and how to utilize the portal when it becomes available.
