This award celebrates outstanding individuals in three categories: our Members (those receiving Care Coordination Services), Family and Caregivers, and Provider Partners.

The awards this year are as follows:
MEMBER RECEIVING SERVICES
– The Trailblazer Award honors a member who receives care coordination from Person Centered Services and has boldly forged new paths through their determination, courage, and vision. This award recognizes an individual who has set a powerful example by breaking barriers, embracing new opportunities, and leading with innovation. The recipient has inspired others by challenging norms, advocating for themselves and their community, and demonstrating a commitment to progress and positive change. Apply now.
– The Growth and Resilience Award celebrates a member who has demonstrated outstanding perseverance, adaptability, and personal advancement. This award honors an individual who, through their unwavering spirit and determination, has overcome obstacles, embraced challenges, and made meaningful progress in their journey. The recipient serves as a role model by inspiring others with their courage, positive outlook, and commitment to self‑improvement and advocacy, embodying the true essence of growth and resilience. COMMUNITY PROVIDER OR PARTNER
– The Community Collaborator Award honors a community provider or partner who has made a meaningful impact on the lives of individuals with developmental and intellectual disabilities. This award recognizes those who go beyond their professional duties to foster inclusion, build partnerships, and strengthen the network of support within the community. Apply now.
– The Person-Centered Champion Award recognizes a community provider or partner who has demonstrated exceptional dedication to supporting individuals with developmental and intellectual disabilities through a person-centered approach. This award honors those who go above and beyond to ensure that each person’s unique needs, preferences, and goals are central to their care and support. Apply now.
FAMILY MEMBER OR CAREGIVER
– The Advocacy Impact Award celebrates a family member or caregiver who has played a transformative role in improving the lives of individuals with intellectual and developmental disabilities. The recipient stands out for their proactive efforts in identifying challenges and championing solutions that benefit their loved ones and the broader disability community. Apply now.
– The Empowered Voices Award recognizes a family member or caregiver who excels at amplifying the perspectives and experiences of individuals with intellectual and developmental disabilities. Rather than advocating on their behalf, the recipient prioritizes empowering those they support to speak for themselves, promoting self-advocacy and self-determination. Apply now.
Anyone who is nominated must know and agree to it in advance. Our members must attach the photo and info authorization form below to be nominated or identified in a nomination. Nominations must be submitted by March 1st.
