Care Connection Day – Hornell

Join us for an exciting in-person networking event where you can connect with local providers and Care Coordinators in your area! This is a free event for all vendors, and attendees, organized by Person Centered Services Care Coordination Organization. Click here to fill out the registration form if you are planning to attend and/or table at one of the Care Connection Day events in your region. Registration is encouraged but not required for general public attendees.
