People with intellectual and developmental disabilities have rights. Care Coordinators can help you navigate these rights if you’re not sure about them.

 

When you receive Care Coordination, you have the right:

To decide on the life you want and participate in a person-centered planning process

To the privacy and security of your Protected Health Information

To access the information you need to make informed decisions about your services

To object and appeal any stated rights, plan of services, care or treatment, and plans for placement or discharge
