Patricia Embraces Change with Team Support
A person we serve received the health care she needed through above-and-beyond support and encouragement.
Care Coordinator Kayla Tedone is dedicated to helping people receive the care they need in the way that is right for them!
Recently, Patricia Cruz, a young woman whom Kayla serves, needed to fast for a medical appointment. Patricia, who has behavioral needs, follows a structured routine that she relies on to live a stable life. She eats pancakes at the same time each morning and counts on this meal to feel safe and at home in the world. Patricia’s family was concerned about the sudden disruption to her routine.
Kayla connected Patricia and her family to Aliya Agins, a behavioral specialist through Presence Developmental Counseling. Aliya shared videos of what Patricia could expect in advance of her hospital visit. She then drove from out-of-state to be there for Patricia on the morning of her appointment. Kayla Tedone met everyone at the hospital to provide additional support and advocacy for the family.
Through a combined team effort, Patricia successfully switched up her routine. Once the appointment was over, she was given her pancake breakfast and an umbrella to add to her personal collection. The family was thrilled that Patricia stayed upbeat throughout the entire experience!
“Patricia has very supportive family that understands her needs and works with her to accommodate them,” said Kayla, who loves partnering alongside them.
Kayla credits Person Centered Services for allowing her to be there for Patricia at the hospital.
“I like the flexibility in where and when I am able to meet people,” said Kayla. “I visit people in different places and at different times, which helps them succeed.”
Kayla continues to connect Patricia to other valuable resources and programs. She is always there to support Patricia, empowering her to reach her potential!
