Agency 101 Spotlight – Care Coordination

An Agency 101 Spotlight on Care Coordination will take place virtually on December 10, 2024 from 12PM-1PM on Zoom. This event is part of a series of spotlight sessions created for families, caregivers, and professionals who work with and support children and students with disabilities. Agency 101 Spotlights provide an opportunity to hear directly from representatives of different agencies regarding eligibility requirements, program options, and the referral process. This session will focus on Care Coordination Organizations and will feature presentations by Person Centered Services and Prime Care Coordination.
