Jeremy Finds Success in Work and in Life

Jeremy Falconer is finding his place in the community he loves. With Care Coordination support, the man now has a job he enjoys and an apartment in Arcade, NY that is perfect for his needs.

Jenna Johnson, a Care Coordinator based in Person Centered Services’ West Seneca offices, has worked with Jeremy for more than five years.

“Jenna helps figure things out,” says Jeremy. “She phones me to see what is going on in my world.”

With Jenna’s support, Jeremy transitioned from full-time prevocational services to a job at Empire Distributions where he now works four days a week. He no longer requires supported employment assistance due to the success and stability he has found at his work.

“Jeremy has not only gained competitive employment by working, but he has also gained a community that is inclusive,” says Jenna.

Empire Distributions has been very accommodating to Jeremy. Around Halloween, the company ensured that arrangements were made so that Jeremy could participate in an employee pumpkin carving contest.

In September of 2021, Jeremy moved out of a certified group home setting and into his own apartment, which has been a long-time goal of his. Jenna connected him to Community Transitional Services funds to furnish his new apartment to his taste.

“I like the location of my apartment,” says Jeremy. “I like walking to restaurants and riding the train. I have a friend in the apartment building and my family is close by.”

Jeremy is flourishing socially, going on neighborhood outings he enjoys as he lives the independent life he has always wanted. His future goals include having a family of his own and buying a car that his support staff can use.

“Jeremy has grown so much,” says Jenna. “It is rare to transition from a group home to the community. Jeremy is always reaching for his goals and will not let anything deter him.”
