The presentation will provide a brief history of Supported Decision Making (SDM) and will address the evolution and current state of Supported Decision Making in New York State. We will discuss how the Supported Decision Making Agreement can be a less restrictive and empowering alternative to guardianship. The presentation will address how a Supported Decision Making Agreement can provide a legal form of support for the Decision Maker and for those with whom the Decision Maker DM enters into agreements. We will walk through the facilitation process that ultimately results in the Supported Decision Making Agreement. Lastly, the presentation will offer additional resources to attendees, as well as how to make a referral to Supported Decision-Making New York.
