– A Care Coordinator will partner with you to create a personalized Life Plan that will help identify which programs, services and supports might be the best fit for you.
– Your Care Coordinator will work to connect you to opportunities in your community that will address the goals and needs you have identified.
– When you are enrolled in opportunities or receiving care, your Care Coordinator will communicate with providers, coordinating your access to health, residential, community and social support services.
– As you experience life changes and transitions, your Care Coordinator will update your Life Plan based on your input. The plan will continuously adapt as new goals and needs arise.
