Overview:

Person Centered Services, a leading Health Home Care Coordination Organization, is proud to strengthen its partnerships with community-based organizations (CBOs) and health care organizations (HCOs) through a newly enhanced collaboration framework. This initiative is designed to foster more effective communication, streamline referral processes, and promote mutual education to better serve individuals with complex needs.

Purpose:

The enhanced relationship aims to:

– Establish Clear Points of Contact: Designated liaisons within each organization will ensure timely and consistent communication, reducing delays and improving coordination of care.

– Streamline Referrals: A unified referral process will be developed to ensure individuals are connected to the right services quickly and efficiently.

– Promote Collaborative Opportunities: Regular meetings, joint initiatives, and shared goals will foster a culture of partnership and innovation.

– Enhance Education and Awareness: Cross-training and informational sessions will be offered to increase understanding of the roles, services, and value of Care Coordination Organizations and community-based partners.
