The Knowledge Center is a tool to help you better understand and navigate the system of supports and services available to people with intellectual and developmental disabilities living in New York State. Please note that “Care Management” and “Care Coordination” are in reference to the same service. In some regions, Care Coordinators are referred to as Care Managers. This tool is designed for state-wide use.
