Robert Finds a Home, Hope, and Friendship
Care Coordinator Lexi Kendall connected Robert Bloom to housing and other supports he needed to lead a safer and more joyful life.
Through personal perseverance and dedicated Care Coordination support from Person Centered Services, Robert Bloom is now flourishing in ways that once seemed impossible!
Robert, who came from the Jamestown, NY area, faced many hardships throughout his life starting at a very young age. His lack of positive relationships led him down a long path of frequent hospital stays and homelessness. Struggling to survive on his own in the community while lacking necessary support led to inconsistent mental health treatment and neglect of Robert’s medical health.
A powerful shift was needed in Robert’s life direction. Over the years, he worked with several Care Coordinators who tried to connect him to opportunities and resources. When he met Lexi Kendall, a Care Coordinator based in the Silver Creek, NY offices at Person Centered Services, he finally experienced powerful breakthroughs that led to greater stability and fulfillment.
Lexi began advocating tirelessly for Robert starting in July of 2025. She partnered closely with the Clinical Services team at Person Centered Services to identify a better course that aligned with his goals. They knew Robert needed adequate housing, specialized clinical treatments, a change in his financial situation, and an escape from the hardships he had faced for so many years.
“My supervisor, Chandra Davis, provided me with a lot of support, which then helped me better support Robert,” said Lexi, who was driven to see Robert succeed.
Because of the assistance he received from Person Centered Services, Robert now lives at an Individual Residential Alternative (IRA) through a provider organization in Angola, NY. He is in a home with all male housemates, who share common interests, and is quickly developing friendships.
Robert now has the necessary circle of support in place to ensure his best quality of life is achieved. He even has a new cat named Gracie! When asked about how he feels about the extraordinary changes in his life, Robert smiled wide and expressed amazement.
“Robert is so happy now!” said Lexi. “He keeps checking in, and every time we talk, it is a positive conversation. Though it can be difficult to find opportunities, if you keep pushing forward things can happen!”
