Spotlight: Jill Guiffre
As a Care Coordinator based in Jamestown, Jill Guiffre goes above and beyond to offer superior Care Coordination that matches the personal needs of each person she supports.
Jill Guiffre has long cared deeply for people, especially those facing hard moments in life. As someone who has experienced times of insecurity and struggle in the past, she has a passion for helping her neighbors and making sure they have the resources they need to survive and thrive.
Given her motivation to make a difference in her community, becoming a Care Coordinator at Person Centered Services proved the perfect role for Jill. As a Care Coordinator, Jill connects people with intellectual and developmental disabilities to care, support, and opportunities that empower them to reach their potential.
“I like connecting on a human level,” said Jill. “I try to have as much patience and understanding as possible. I want the members I serve to see me as a real person.”
With a degree in Mortuary Sciences, Jill began her career in funeral services. This provided her with an empathetic perspective on death, dying, and resilience. She later offered support in complicated grief cases at a residential treatment facility.
Jill has also held positions in business, social work, and tech. Her glimpse into so many different sides of life gave her insights that have let her better serve people on her caseload.
“No two people are ever the same,” said Jill. “Our caseloads are diverse. We need to be diverse ourselves in response. I think of Care Coordinators as Swiss Army Knives, capable of so many things.”
Jill is proud to have helped people who were in and out of emergency housing, securing them with better living conditions. She has provided them with transitional assistance, including toiletries and hygiene products.
“Everyone should be able to stay clean,” said Jill, who has a knack for finding low-cost, high-quality goods.
Jill is well-known for collecting and stocking items to donate and share. She is always looking out for people on her caseloads, as well as members in her greater community.
When SNAP (Supplemental Nutrition Assistance Program) benefits were on hold during the government shutdown, she went out of her way to connect local people to resources, coordinating with her neighbors to support those with the greatest needs. She intentionally distributed items in a strategic and coordinated way. Her impact during a time of crisis was truly remarkable.
Jill also runs a “tote ministry” where she donates a tote full of goods to families in need. She further takes part in “Toys for Tots” during the holiday season.
“I am big on the Golden Rule,” said Jill. “I never forget where I came from and times when I didn’t know where to turn.”
Jill is always there to encourage and support her fellow Care Coordinators in Jamestown.

Jill Guiffre also collects coupons from local Tops stores and seeks opportunities in local sales flyers.
“My biggest takeaway I share with other Care Coordinators is to think outside of the box,” said Jill. “Keep going further. Roll your sleeves up! Our work really does matter. We really are impacting people’s lives.”
