Person Centered Services is dedicated to promoting the best health outcomes for those we serve, identifying and connecting people to life-changing health care options.
– Care Coordinators coordinate all of your health needs, including checking on appointments and transportation, and identifying health issues.
– Your Care Coordinator can help you look at the “big picture” and figure out strategies to address areas of your life and health that you may want to improve.
– Person Centered Services also has a comprehensive Clinical Team consisting of Registered Nurses, a Behavioral Health Specialist and a Mental Health Professional who offer support and expertise.
– Partnering with the Clinical Team, Care Coordinators are able to assist you with managing chronic health conditions to improve overall health and quality of life.
