Diane Achieves Better Health and Greater Independence

Although it has been a long road to independence, Diane Flanagan is now living her best life with support from her Care Coordinator, Elisabeth Nagel!

Elisabeth had coordinated Medicaid Services for Diane through her position at a previous organization. While working at Person Centered Services, she was asked to cover Diane’s case again during the COVID-19 pandemic. Since then, Elisabeth has continued to support Diane, which has proved transformative and life-changing.

“When I first met Diane before transition to Person Centered Services, she was in the middle of many difficult situations,” says Elisabeth. “At 65 years old, she had never lived in her own apartment independently. She also was not in close touch with most of her siblings.”

Diane has now lived in her apartment for close to six years on her own. Three of her siblings are involved in her everyday living and continue to support her success in the community. Diane has developed better eating habits and has increased her exercise, which is greatly supporting her health as a person with diabetes. Diane walks at least two miles a day and uses exercise machines during days with bad weather.

“Diane is doing excellent,” says Elisabeth. “She is keeping to a routine and has developed a great system that works for her. Diane has done very, very well while living on her own. She has reconnected with a lot of family.”

Over the years, Elisabeth has helped Diane access the Supplemental Nutrition Assistance Program (SNAP) and adjusted her insurance type to make sure her medication is more affordable. She checks in with Diane regularly to ensure that she can connect to the programs and services she wants and needs.

“I love everything about working with Elisabeth,” says Diane. “She’s done everything for me.”

Beyond the improvements to her physical health, Diane has also experienced improvements in her personal life. She enjoys completing challenging puzzles and travelling across the county with friends and family. Diane is especially proud of her children and grandchildren, and loves the closer relationships she now has with her brothers and sisters.

“Diane is a go-getter,” says Elisabeth. “She has the desire to be independent, but also knows she has people she trusts in her circle of support.”
