Care Connection Day – Rochester

Join us for an exciting in-person networking event where you can connect with local providers and Care Coordinators in your area! This is a free event for all vendors, and attendees, organized by Person Centered Services Care Coordination Organization.

Click here to fill out the registration form if you are planning to attend and/or table at one of the Care Connection Day events in your region. Registration is encouraged but not required for general public attendees.

Care Connection Days will be held on the following dates:

• The address for the event is the The R. Thomas Flynn Campus Center (Warshof Conference Center) at 1000 East Henrietta Road Rochester, New York 14623.

• Follow the signs for Building 3 (the R. Thomas Flynn Campus Center) which can be found in the rear of the campus. 

• Turn on to Campus Road, which is next to a pond. We recommend that attendees park in Lot N or M, or in any open, non-metered parking lot or space that is not marked for “employee parking only”.

• Enter Building 3 using the NORTH entrance, which is near the pond, and then go to the 2nd floor using either the stairs or the elevator. Monroe A and B is at the top of the stairs.
