Care Connection Day – Hornell

Join us for an exciting in-person networking event where you can connect with local providers and Care Coordinators in your area! This is a free event for all vendors, and attendees, organized by Person Centered Services Care Coordination Organization. Registration is encouraged but not required for general public attendees.
