Coronavirus Update: Information information on Care Coordination for Individuals, Families & Caregivers

To Individuals, Families and Caregivers:

As we continue to follow the most recent circumstances regarding Coronavirus, Person Centered Services knows how important it is to keep you and/or your loved one in regular touch with your Care Coordinator.  We are doing everything in our power to provide uninterrupted, high-quality Care Coordination while at the same time keeping your health in mind, as well as the health of our employees.

If you were scheduled to meet in person with a Care Coordinator within the next several weeks, that meeting will likely be handled via computer technology or telephone. The details of how that meeting will be set up will come from your Care Coordinator, who will be contacting you soon. That is the only change to Care Coordination that we are applying at this time. All other Care Coordination will remain the same.

We want to be sure we can protect, serve and support you. Our Care Coordinators are here to assist you with measures to prevent contracting and spreading the virus, such as effective handwashing and avoiding others who are sick. Your Care Coordinators will also be reaching out to you frequently to check on your health and wellbeing and to determine how they can help, if necessary.
