Thank you for choosing Person Centered Services Care Coordination Organization for your Care Coordination Services! We look forward to helping you through the intake and enrollment process. An Intake Specialist will work closely with you to ensure the process goes smoothly and quickly, from beginning to end.
Using the buttons below, you will find links to forms and notices for the regions we serve. Individuals served in Cayuga, Chemung, Livingston, Monroe, Ontario, Schuyler, Seneca, Steuben, Wayne and Yates counties should use the East Region Referral Packet. Individuals served in Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, and Wyoming counties should use the West Region Referral Packet.
