What can I expect from Person Centered Services during the COVID-19 pandemic?
Our Care Coordinators are committed to serving you during this crisis. We are here. We are fully open and running. Please be advised that in the coming weeks your Care Coordinator will be reaching out to you more frequently than normal to monitor how you’re doing during this crisis, to be sure that all is well, and to connect you to any services that might be necessary.
What if I need to see my Care Coordinator in person?
If you were scheduled to meet in person with a Care Coordinator within the next several weeks, that meeting will likely be handled via computer technology or telephone. The details of how that meeting will be set up will come from your Care Coordinator. Learn more about how we determine cases where in-person support is necessary here.
What if my Care Coordinator gets sick and can’t work with me?
We will help provide you with a replacement or interim Care Coordinator and make sure you continue to receive the support you need.
What if I need to reach my Care Coordinator and do not have the number?
Call 1-888-977-7030 between 8:00am and 4:30pm or e-mail us at email@example.com. Someone will ensure that you have the number to your Care Coordinator and your Care Coordinator’s Supervisor. If you are already enrolled and have an urgent need between 4:30pm and 8:00am, our on-call service can be reached at 1-833-200-0678.
