How long have you been with Person Centered Services?

I have been with the organization since its start in 2018. It has been an amazing journey!

How did you first hear about our organization?

I transitioned from a Medicaid Service Coordinator role at my previous employer to a Care Coordinator position at Person Centered Services as part of New York State’s system-wide reorganization.

Previously, I worked with providers like People Inc. and Heritage Christian Services, where I gained a broader perspective on how programs and systems for people with intellectual and developmental disabilities operate.

What is your favorite thing about working at Person Centered Services?

My favorite thing about Person Centered Services is the relationships I am able to build, both with the people I support and my fellow team members.

Since I’ve been with the organization from the start, I’ve remained connected to certain individuals for years. The long-term partnerships I’ve been able to foster with teammates have been extraordinary. We lean on one another for support.

How does your work help Person Centered Services thrive?

My long-time experience and deep institutional knowledge have helped me empower others. I’ve been able to directly transform lives by connecting the people I support to self-directed services, mobility equipment, and home modifications that make their living environment safer and more accessible.

I have so many connections to providers in our community that I leverage to make sure people receive the supports and services they deserve. These invaluable relationships were forged over time. It is easier to navigate the system when you have knowledge of local resources and connections.

I also love sharing my knowledge with my teammates and learning from them. It helps strengthen not only our individual roles but also the team as a whole. I try to make sure that others always have a voice to listen to them, and to offer advice or guidance when desired.

The work we do is so critical, and it is always inspiring to see our impact!

What do you enjoy doing outside of work?

I love volunteering through Sadie’s Safe Harbor Canine Rescue, where rescued dogs are matched with foster families in the Western New York area. I’m proud to foster dogs myself and to champion opportunities for others to provide safe homes for animals.

I also enjoy baking and making treats! I have an 11-year-old daughter whom I am always supporting and encouraging, and I especially enjoy baking for her.

What advice do you have for other Care Coordinators?

I encourage Care Coordinators to connect with others in their pod. They should feel comfortable asking their fellow team members questions and collaborating in smaller groups.

New Care Coordinators can learn so much about the role by connecting with experienced team members who can support and mentor them.

Person Centered Services is grateful for the contributions of Care Coordinators like Holly and all they do to help the people we serve succeed!
